Diagnostic Intake Record
Capture the minimum whole-case context before assigning a cause.
Can another grower understand what changed without seeing your proposed diagnosis?
Symptom Distribution Map
Describe where symptoms occur so location becomes diagnostic evidence.
Does the mapped pattern support more than one plausible cause? Keep those alternatives open.
Environment Snapshot
Record the environment where the affected tissue actually exists, not only a room average.
Are the measurements from the plant zone that shows the problem, with units and locations preserved?
Irrigation & Root-Zone Record
Separate water-balance, aeration, salinity and root-condition evidence from leaf appearance.
Could another person reproduce your pH, EC and moisture measurements from the method notes?

Make corrective actions testable instead of stacking several changes and losing causal information.
If the plant improves, will the record tell you which change likely mattered?
Follow-Up Verification & Case Closeout
Decide whether the interpretation survived follow-up evidence and preserve what was learned.
Did the follow-up test the original interpretation, or only document that time passed?

Record facts separately from interpretations.
A record improves diagnosis only when observations are separated from interpretations. Preserve units, sample location, time, plant stage, recent changes and uncertainty. A completed worksheet does not by itself confirm a pest, pathogen, nutrient deficiency or environmental disorder.
